Emergency GI Bleeding

GI Bleeding Treatment in Jalandhar — Emergency Endoscopic Care

Blood in vomit or stool is never something to watch and wait on. Endoscopic haemostasis, variceal banding and a 24/7 Gastro ICU are available at Innocent Hearts Hospital, Nakodar Road.

Quick answer

Black, tarry stools mean upper GI bleeding — blood digested on the way down — and need endoscopy within 24 hours. Bright red blood is usually lower GI, most often piles, but polyps, IBD and cancer present the same way. Any blood, even once, needs assessment. Endoscopy both finds the source and stops it in the same procedure.

Upper or Lower — the Colour Tells You

Where the bleeding comes from changes the urgency, the test and the risk. The appearance of the blood is the first clue.

Upper vs Lower GI Bleeding

Two different problems that patients describe with the same three words. This is how they are told apart.

Oesophageal varices are dilated veins that form when cirrhosis raises pressure in the portal vein. A first variceal bleed carries a 15 to 20% mortality. Emergency variceal banding controls the bleeding in over 90% of cases — which is why cirrhosis patients are screened before anything happens, not after.

Emergency Protocol

What Happens When You Arrive Bleeding

A fixed sequence, because in a bleed the order matters more than anything else.

1

Stabilise

IV access, fluids, blood products where needed, and continuous monitoring. Nothing diagnostic happens before the patient is stable.

First
2

Risk stratify

Scoring identifies who needs endoscopy within hours and who can be scoped the next morning — so the sickest go first.

Triage
3

Upper endoscopy

Finds the source and treats it in the same sitting: argon plasma coagulation, haemoclips, adrenaline injection, or variceal band ligation.

Within 24 hrs
4

Lower GI evaluation

Colonoscopy once stable, with polypectomy or banding as the findings require.

After stabilisation
5

Remove the cause

H. pylori testing and a 14-day eradication course, NSAID review, and beta-blockers plus repeat banding for varices.

Prevents recurrence
Why This Matters Locally

A Bleed Is Not Something to Drive to Another City With

The equipment and the ICU have to be in the same building as the endoscopist, at two in the morning as much as at two in the afternoon.

Where variceal bleeding really starts

The most dangerous GI bleed is a liver problem wearing a stomach disguise

Oesophageal varices do not form on their own — they form because cirrhosis has raised the pressure in the portal vein, and a first variceal bleed kills 15 to 20% of patients. The genuine prevention is upstream, in liver care: FibroScan, treating the cause, and screening before a bleed happens. That upstream work is also where Hepatolean Advance Liver Pro+ came from, the sugar-free liver-care protein Dr. Ankit formulated for his fatty liver patients. It has no role in an active bleed — that is an emergency, and the only correct response is the hospital.

Patient FAQs

GI Bleeding — Frequently Asked Questions

Is blood in stool always serious?
It should never be ignored, even when it looks minor. Small amounts of bright red blood most often come from piles, which is not immediately dangerous but still needs evaluation. The same appearance can also mean polyps, cancer, IBD or upper GI bleeding. Any blood in stool, even once, warrants a consultation — and a colonoscopy if you are over 40.
What causes black tarry stools?
Melena means bleeding in the upper GI tract. As blood travels down the intestine it is digested and turns black. It is most often caused by peptic ulcers, frequently from H. pylori infection or NSAID overuse, or by variceal bleeding in cirrhosis. It requires urgent upper endoscopy, ideally within 24 hours.
What is H. pylori and how does it cause bleeding?
It is a bacterial infection of the stomach lining affecting roughly 60 to 70% of Indian adults. It destroys the protective mucus layer, letting acid erode the stomach wall and form peptic ulcers, which can bleed — sometimes severely. A breath test or a biopsy during endoscopy diagnoses it, and a 14-day antibiotic course cures it permanently.
How quickly can bleeding be diagnosed?
Upper GI bleeding is diagnosed within hours by emergency endoscopy, which also treats the source in the same procedure. Lower GI bleeding is evaluated by colonoscopy once the patient is stable. Urgent endoscopy is available 24/7 at Innocent Hearts Hospital.
What are oesophageal varices?
Dilated veins in the lower oesophagus that form when portal hypertension from cirrhosis forces blood through alternative vessels. When these thin-walled veins rupture the bleeding is massive and life-threatening — a first bleed carries 15 to 20% mortality. Emergency banding controls it in over 90% of cases.
Can GI bleeding be treated without surgery?
In the large majority of cases, yes. Endoscopic haemostasis — argon plasma coagulation, haemoclips, adrenaline injection, band ligation — stops most bleeds without any incision. Surgery is reserved for the minority that endoscopy cannot control.
The bleeding seems to have stopped. Should I still come?
Yes. A bleed that stops on its own has not been explained, and the underlying cause — an ulcer, a polyp, varices, a cancer — is still there. Stopped bleeding buys you time to be assessed properly; it is not a resolution.
